Do you need to request a medication review?

A medication review is a one-on-one review of your medications.

We would like to work with you to ensure that your medications are doing what they should – and if they are not, we will make a plan to fix that! This is also a great opportunity to answer any questions you might have about your prescriptions because your well-being is our priority. 

Are your medications doing what they should?

A medication review is just a meeting with a pharmacist, in which we’ll review your current prescriptions and medical history. You can let us know about any issues or questions you might have, and we’ll work together to get everything working the way it should.

Step 1: Request a medication review.

Requesting a review is easy. Simply call the pharmacy or fill out the form here and we’ll get started! You’ll need to gather a few things:

  • List of all medications and name of prescribing physician – this includes mulit-vitamins and over the counter items
  • Note any issues, symptoms, or concerns

Step 2: Meet with your pharmacist.

During this time your pharmacist will review each medication with you to make sure you understand what you are taking, why you are taking it, and we will make sure there are no therapy duplications or interactions.

We also can discuss compliance aids such as compliance packaging and can make suggestions on how to make taking medications easier. Our goal with a Medication Review is to make sure you understand the importance of each medication and why it is necessary for you.

Step 3: Make a plan.

We will notify the patient, at the time of the Medication Review, what issues may need to be discussed with their physician.

What is the purpose of a Medication Review?

The purpose of a medication review is to get you 1:1 time with your pharmacist to ensure there are no possible medication errors, discuss any side effects, and to improve your medication experience.
